Request Health Records

At The Aga Khan University Hospital, we maintain every patient’s medical record in confidence.  This record captures certain information about the treatment (investigation report) you may have received at our hospitals and can be used for medical reference in the future. We also issue birth certificates, death verification letters and any amendments to these documents.

All patients who have received care at The Aga Khan University Hospital, Main Hospital Campus, Karachi and the Clifton Medical Services, Karachi, can retrieve copies of medical records, birth certificates and death verification letters from our Health Information Management​ Services (HIMS). Documents can be collected in person, or requested online through a courier service, for which there is an additional charge. 

Patients who have received care at any one of our other community hospitals can retrieve copies of their medical records, birth certificates and death verification letters directly from the hospital.

​Identification documents will be requested from the patient, family member or attendant to verify that they are authorized to have the access to records.​

  • ​All Reports: Patient, family member or attendant will need to submit copy of the payment receipt, white plastic hospital registration card, CNIC and/or Passport.

  • Birth Certificate: Either parent will need to submit the acknowledgment receipt. If the parents are unavailable, then the family member or attendant must submit a letter authorized by the parents, and show their original CNIC and submit a copy.

  • Death Certificate: Immediate family members including mother, father, son, unmarried daughter, spouse will need to show their original CNIC and submit a copy. If family members are unavailable, then the attendant must submit a letter authorized by an immediate family member.​​


​All patient reports less than 3 months old are provided free of cost and for all reports older than 3 months will be charged.

  • All reports PKR 20/ per page
  • Original Birth Certificate PKR 2500/-
  • Birth Certificate Amendment PKR 1400/-
  • Birth Cetificate Duplicate PKR 1400/-'
  • Death Verification Letter PKR 1400/-

Identification documents can be submitted both in person and online. You can submit the documents in person at the AKUH Main Campus, Karachi, HIMS Release of Information counter, ground floor of Ibn e Zuhr building, between 9 AM to 8 PM from Monday to Saturday.
