At The Aga Khan University Hospital, patient medical records are maintained with strict confidentiality. These records include information related to investigations and treatment received and may be required for future medical reference.
We also facilitate requests for birth certificates, death verification letters, and related amendments.
Accessing Your Records
Patients who have received care at the Main Hospital Campus (Karachi) or Clifton Medical Services can request copies of medical records, birth certificates, and death verification letters through Health Information Management Services (HIMS).
Requests can be:
Submitted and collected in person, or
Requested online, with delivery via courier (additional charges apply)
Patients who have received care at off-campus hospitals can obtain their records directly from the respective hospital.
